What Happens Next

1. We start with a conversation

Tell us about your situation, your DOL White Card status, and your current health needs. There's no pressure and no paperwork required for this first call.

2. We review the next best step

Our team will confirm what services are covered under your White Card and help you understand what a care plan could look like for your situation.

3. We build your care plan

If Legacy is a good fit, a registered nurse will visit your home to complete an assessment and get care started under your approved benefits.

Common Questions

Quick answers before you reach out.

Do I need every document ready before I contact you?+

Not at all. The only thing that's truly helpful to have on hand is your DOL White Card. We'll guide you through anything else during our first conversation - there's no need to prepare anything in advance.

Can I contact you for a parent or spouse?+

Yes, absolutely. Many of our calls come from adult children or spouses who are helping coordinate care. We're happy to speak with you directly and walk your family through next steps together.

Do you only help people who are already approved?+

Our in-home care services are designed for individuals who hold a DOL White Card. If you're not sure whether you or a loved one qualifies, give us a call and we'll help you figure out the best next step.
